My child, as listed above, has my permission to participate in the North County Fastpitch Program. To the best of my knowledge, he/she is in good health and able to engage in this type of activity. The league has my permission to admit him/her for emergency treatment if it becomes necessary. I do hereby affirm that I will not hold the North County Fastpitch Association, nor its officers, managers, coaches, sponsors or any other team liable for any injuries, damages or expenses arising as a result of these activities.
